We're excited to help you get started with the enrollment process. Below, you'll find all the information you need to enroll your child(ren).

Student Enrollment Forms

Enrollment Checklist

To complete enrollment, please have the following documents ready:

  • Proof of Menomonie Residency – Provide one of the following documents with your name and current address:
    • Current utility bill
    • Rental agreement
    • Mortgage statement
    • Property tax bill
       
  • Immunization Records – If these are not transferred from your child’s previous school, please include them with enrollment. You can request records from your healthcare provider or check the Wisconsin Immunization Registry.
     
  • Additional Documents (if applicable) – Include any relevant paperwork, such as:
    • Custody or legal documents

  • Once you've completed the forms, please email the enrollment packet, proof of residency, and any additional documents to:

  • Stay connected! Help us keep our records up to date by filling out our Census Google Form today. This form helps us plan for future enrollment and gather information from families with younger children who are not yet enrolled, as well as any students living in the district who are not currently attending.

  • For elementary students, school placement is based on your residence. Use our District Boundary Map to determine your assigned elementary school. Additionally, the district has one middle school building (grades 6-8) and one high school building (grades 9-12).

  • For students entering grades 9-12 after the school year has begun, please send the student's transcript along with the enrollment packet if able. This will help us begin scheduling classes and determining an ideal start date.
