Download and save the packet to your device or print it. Complete pages 2-8 of the fillable packet, save again to your device, and submit these pages using one of the following methods listed below.

4K Packet for the 2026-2027 School Year

Please note:  The last two pages of the enrollment packet include Indicators of 4K Readiness and a Family Observation Checklist. This information is being included for families to help identify skills they can be working on in preparation for student success in a 4K classroom. This information does not need to be returned.

HOW TO SUBMIT YOUR 4K ENROLLMENT

ONLINE ENROLLMENT (PREFERRED METHOD)
Complete the required forms electronically using the directions above

  1. Save the completed forms
  2. Email the forms to enrollment@msd.k12.wi.us
  3. Attach the required documentation as shown below to the email, or bring the documents to the district office located at 215 Pine Avenue E., Menomonie, WI

IN-PERSON

    You may also deliver the completed forms to the Administrative Service Center during regular  business hours: Monday through Friday, 8:00 a.m. to 4:30 p.m.


REQUIRED DOCUMENTATION

To complete the enrollment process, please submit the following items along with pages 2-8 of the 4K packet:

  • Proof of Age: Child’s birth certificate
  • Immunization Record: See page 7 of the packet and type in the dates in Step 2 or attached a printed record
  • Proof of Address: A piece of mail or bill showing parent/guardian’s name and current address
  • Additional Documents: (if applicable) Court or legal custody documentation

If you are not a resident of the School District of the Menomonie Area and would like to inquire about open enrolling your child into our 4K program, please click Public School Open Enrollment Wisconsin Department of Public Instruction. The open enrollment application period for the 2026-2027 school year is from February 2, 2026 until 4:00 p.m. on Thursday, April 30, 2026.  To complete the open enrollment process that opens on Monday, February 2, 2026, please click Open Enrollment Application Process 2026-2027.
