4K Program

Do you have a child who will be 4 year old on or before
September 1, 2023?
If yes, enrollment for the 2023-2024 School Year will begin on Monday, February 6, 2023, and will continue throughout the school year. Please click on the pencil or 4K packet link below to start the enrollment process.

4K enrollment following the directions below.


4K packet for the 2023-2024 School Year
Submit the required forms using one of the following three methods.

 1.  Online enrollment is the preferred method. Complete the required forms electronically by simply following the directions and attach your child’s immunization records, proof of residency, and birth certificate to your digital enrollment.

       Save again and email to: [email protected]

           2.  Mail Paper Copies to:      Administrative Service Center
                                                        Attn: 4K Enrollment
                                                      215 Pine Avenue E.
                                                      Menomonie, WI  54751

3.  Bring completed forms to the Administrative Service Center (ASC). Our office hours are 7:30 AM to 4:30 PM, Monday through Friday.

To finalize the enrollment, please submit

pages 2-8 with the following items.

*  Proof of Age (child’s birth certificate)
*  Proof of Address

*  Other important documents, if applicable (court/legal custody documentation)
