KG-12th Grade Enrollment
Dear Families,
We're thrilled to welcome you to the Menomonie Area School District!
You will need the following items ready for when you enroll:
Enrollment Checklist
- Proof of Menomonie Residency: Ensure you have one of the following documents, which must include the current address and name of the parent or guardian: a current utility bill, rental agreement, mortgage statement, or property tax bill.
- Immunization Record: If your immunization records are not transferred from the previous school, please include them with the enrollment. You can get these records from your healthcare provider or check the Wisconsin Immunization Registry at
https://www.dhfswir.org/PR/clientSearch.do.
- Additional Documents: Include any relevant documents like an IEP or custody/legal documents if applicable.

For students in elementary grades, your assigned school is based on where you reside. You may access the District Boundary Map to find out your boundary school.
Students entering grades 7-12 should send their transcripts at the time of enrollment to help us begin scheduling classes and selecting the ideal start date. We'll still request records from the previous school.
